Immunization Requirements

Walk-In Immunization Clinics: Potter and Randall Counties

• Take a current shot record to all locations.
• Children under 18 must be accompanied by a parent or legal guardian.
• Call ahead to ensure location has supply of vaccine needed,
• Age restrictions may apply at some locations.
• Clinics charge for private vaccine, some clinics are able to bill insurance.
• Each family must know their insurance benefits prior to visiting the clinic.
• This is not an all inclusive list; please contact any pharmacy or your doctor’s office.

Walk-in clinic: Monday-Friday 8-11:00 & 1-4:30.
Extended hours: 1st Tuesday of each month 8-11:30am & 1-6pm
Cost: $5 for one shot, $10 for two or more. No eligible patient will be turned away due to inability to pay.
Children: Medicaid, No insurance, underinsured Adults: No insurance, Medicaid, underinsured
All patients must bring a current shot record. No private insurance accepted.

Walk-in clinic: Monday-Friday 12-6pm
Cost: 22.00 for the first vaccine & $15.00 for each additional vaccine after that Children: Medicaid or No insurance

Walk-in clinic: Monday 8-6 (closed 12-1) Tuesday-Friday 8am-8pm (12-lclosed)
Cost: $22.00 per visit
Children: Medicaid (does not accept Amerigroup), CHIP, No insurance
All patients must bring a current shot record.

Grand Pharmacy• 3500 NE 24th • 350-7455

Walk-in clinic: Monday -Friday 9am -7 pm
Cost: $22.00 per vaccine
Children: Medicaid, underinsured

Walgreens, CVS, United and other pharmacies provide vaccine. The cost varies. Please call in advance for clinic times and prices.
